Near Miss

Place, Hazard, and First Action on an HSE Record

The HSE form asks for place, hazard, and first action. Near miss is a record type on that form.

An HSE record keeps workplace hazards and events tracked inside the organization. The form is one page. Place, hazard, and first action are the spine of the file. The file does not close on an empty hazard line; the reviewer asks for revision. HR and HSE explain form language in training as situation language, not blame. Early reporting keeps the measure. Early reporting keeps place detail. The checklist is written into the period agenda. The reference number does not change.

Near miss is a record type inside the HSE root. It is not a separate root module. Menu and marketing address tie to the HSE page. The reference prefix belongs to the HSE family; it does not mix with the suggestion list. Suggestion benefit type is not carried onto this form; classification here comes from type and the hazard line. The type filter is fixed on the list. The type list is updated from administration. The dashboard total shows open and closed counts. The place line is written at line or rack level.

Field guidance starts on the place line. A general word such as workshop is not enough. Line, corridor, rack, or room is written. In a multi-branch structure the record binds first to company, then to branch. Branch selection completes the place information. If branch is empty, period frequency shifts. Location is the branch place field; there is no separate location box on the form. Records with an empty branch are listed apart. Role decides the visible task row. The frequency table is read from the type filter.

Hazard describes the risk that was seen. A person's name is not required; the event itself is written. Missing personal protective equipment, a slippery floor, a close pass on a forklift path, or an open chemical lid enters this line. Situation language is used instead of blame language. A round there-is-a-hazard sentence is not enough to close. If a measure exists, the figure is added to the line. Situation language is reinforced in training. Step history carries a time stamp.

First action is the decision taken on that shift. Pulling warning tape, closing an area, or lowering a load is first action. A permanent rail, floor line, or procedure change is separate work. A suggestion or kaizen is opened when needed; a link record leaves a trail. Immediate action and permanent work are not mixed in one sentence. On a night shift the spine is the same. Permanent work is tracked on a separate file. The branch filter keeps period comparison.

Reviewer checks run in order. Is the place line specific enough. Is hazard in situation language. Does first action belong to that shift. If a photo exists it opens from the attachment area. A close without rationale is not accepted. If the hazard repeats, the root-cause line is read. A missing field is named in the reason. Check order is fixed in the training note. An attachment binds to the record.

Common mistakes enter the period agenda. Writing a near miss on the suggestion form. Leaving only a branch name instead of place. Packing a permanent project description into first action. Opening an unmeasured kaizen and skipping the HSE close. These mistakes break the frequency table and the suggestion report. The training note is renewed with sample place lines. The mistake list is renewed once a month. A decision without rationale is not accepted.

A photo is attached when one exists. The attachment stays in the dedicated file area. Review is read from the same file; a board sheet or a chat screenshot is not the source of truth. Sign-in is through the browser over SSL. If an attachment is removed, decision rationale stays on the file; missing attachment does not cancel writing a reason. Without a photo, place and hazard text must be specific. SSL sign-in is required. The menu hides a closed screen.

Visibility defaults to internal. A public showcase stays closed for this record type. An authorized HSE reviewer opens the task from the list. A comment is a note from a second person who saw the event; it does not stand in for the decision. The task list opens with the HSE filter; the suggestion queue does not mix in. Role hides a row without permission. A comment does not stand in for the decision. The unit name is written in company language.

A repeating hazard can make the root-cause line required. Quality and HSE comment on the same file. At close, the outcome note states whether the first action became permanent work. If it did not, a new record is opened; older place and hazard are read, not copied. A success story may be written into the period summary when permanent work was applied. Copy-paste does not fill the branch field. The reference number does not change.

Step durations may carry separate day counts for HSE. Defaults for technical decision, author revision, and manager revision sit in company settings. The due date is written when the task opens. The overdue badge looks at the same date. Reminders land in the portal inbox. Being overdue does not justify a close without rationale. The overdue badge looks at the same date. The hazard line is kept in situation language.

The list filter looks at record type. Near miss frequency is produced from branch and place information and is carried into the HSE agenda. HR sees silent units and ties participation to the training plan. The period meeting looks at type spread first, then the unit row. No forecast sentence is produced. The dashboard shows frequency row by row. A silent unit is tied to the training plan. A silent unit is a zero near-miss row.

Kaizen opens for a correction that needs a measure; it does not replace a near miss. A suggestion is an improvement idea not yet applied; an injury-free event is not written on a suggestion. Modules do not copy each other's fields. A link keeps a trail between two files; it does not put them in each other's place. Benefit type is not on the HSE form. A separate file is opened for permanent work. Suggestion benefit type does not enter this form. Role decides the visible task row.

Awarding points is a company decision. Without a rule, near miss creates no points. With a rule, event and recipient are written separately. When a catalog request opens, points are held; they finalize on fulfillment and refund on refusal. Points do not stand in for the accuracy of the hazard line. A notice does not write points. The balance does not go negative. Points do not verify the hazard line. Step history carries a time stamp.

Training explains that the form is not a blame tool. An early near miss is a record that prevents injury. A late report loses measure and place detail. The portal leaves one door. The demo portal may show a sample HSE record; live production data is not in the sample. Demo rows do not move into the frequency report. The demo is not a frequency report. The branch filter keeps period comparison.

An audit asks for reference number, type, place, hazard, first action, step history, and decision rationale. An event list on a shared spreadsheet does not replace the source. Excel export carries the on-screen filter; a person without permission cannot export. The decision sits on the portal file. A sample file is added to the audit pack. The audit pack carries a sample file. An attachment binds to the record.

At setup, if the HSE module is open, the type list, roles, and day counts are written. An unused type does not sit on the menu. Before go-live, the unit and branch lists are completed. Company then branch is required. Price and calendar are clarified on the contact form. SSL protects sign-in and the form. An unused type does not sit on the menu. A decision without rationale is not accepted.

The place line is reinforced in training with examples: paint line three, forklift corridor B, chemical rack two. A branch name does not stand in for place. On a repeating slippery-floor event the root-cause line is filled. A cleaning plan or floor-material note enters the close. If there was no conversion, a new record opens by reading the older place. The menu hides a closed screen.

On the HSE agenda frequency is read by branch and place breakout. A silent unit is tied to the training plan. The suggestion list does not mix into that agenda. An auditor looks for place-hazard-action order on a sample file. At morning handover the need for permanent work is written separately; a link or a new suggestion is opened at that point. The unit name is written in company language.

In short: an HSE record opens with place, hazard, and first action. Near miss is a type on that form. Permanent work is written on a separate suggestion or kaizen. Company is chosen, then branch. Frequency and close are read from the same list.
