Near Miss Records: Risk and Immediate Action

A near miss is an incident without injury. Risk and immediate action are written on the same record; visibility is internal.

A near miss is an event that closes without injury. The form asks for place, risk, and immediate action. A wet floor, a broken warning tape, a close pass on a forklift path, or an open chemical lid is written in this module. A photo is attached when one exists.

Immediate action is the decision taken on that shift. A permanent rail, floor line, or procedure change is separate work; a suggestion or kaizen is opened when needed. The two jobs are not mixed in one sentence.

The module default is internal. A public showcase is off for this record type. The HSE reviewer sees the record on the task list. If the risk field is empty, revision is requested; the file does not close.

The reference prefix belongs to the near miss module. It does not mix with the suggestion list. A hazard spoken at handover vanishes the next day if it is not written. The portal leaves one door.

A repeating risk makes the root-cause line required. Quality and HSE comment on the same file. A comment is a note from a second person who saw the event. A question returns a missing field to the author.

Missing personal protective equipment is written without requiring a person’s name. An unbalanced load on a warehouse rack enters the record before a fall. At close, the outcome note states whether the immediate action became permanent work.

A web-based form replaces the paper minute folder. The list filters by location; branch frequency is read from the period list. HR sees silent units and ties participation to the HSE training plan.

The category list is set from company risks such as slip, collision, energy, and chemical. Corporate identity is the same logo and color on this screen as well. Staff do not see a foreign form.
